Long Term Care

woman with fatherLong-term care is available for those who can no longer live safely in their own home, need medical management for chronic health conditions, and/or help with the activities of daily living. We offer skilled nursing care and a full range of long term care services.

Many of our long-term residents find that their quality-of-life improves after a move to Grace Barker. The social interaction with others and the healthful meals can make all the difference.

We also serve residents who have Alzheimer's disease/dementia in a safe, secure environment where we strive to honor and engage each person’s abilities. Residents with memory loss participate in many of the activities and social events that we offer. The full range of services and amenities is available to residents with these conditions.

Hospice care is provided for residents who have a life-limiting illness.
